Juuso, a game producer, has some sound and straight-forward advice for artists when they send emails for potential positions. Of course, the structure really applies to any position you're in! It just varies depending on what kind of portfolio you have and how you word your email.

Thank you for sharing. The only problem I find is that so many companies make finding out people's names so difficult...

A trick I use is calling the company front desk and asking who the hiring leader is from there I will know who to send emails to. At times yes they will mask it but if you can get to a directory and start calling ppl down the line odds are one of them will pick up and you will get the info you need. Just be honest and strait forward with who you want to talk to.
